Washington - A woman in her 30s died of swine flu in Texas, state health officials confirmed Tuesday, taking the overall toll from the outbreak in the United States to two.

The woman lived in Cameron County, near the US-Mexico border, and also suffered from other chronic health problems, said Carrie Williams, a spokeswoman for the Texas Department of State Health Services.

The woman died last week, Williams told the German news agency dpa. Last week, a 23-month-old boy from Mexico City died in a Houston hospital.

The US Centres for Disease Control and Prevention (CDC) said Tuesday that the number of confirmed cases in the US increased to 403 in 38 states. New York reported 90 cases, Illinois 82, California 49, Delaware 20 and Texas 41.

Health and Human Services Secretary Kathleen Sebelius, who visited the Atlanta, Georgia-based CDC on her first official trip outside Washington, said there were "another 700 probable cases in 44 states."

"We know there will be more cases. The numbers will go up and unfortunately there are likely to be more hospitalizations and more deaths," she said.

Monday's US count was 286 cases in 36 states.

"But the good news is that the virus does not seem to be as severe as we once thought it could be, based on the very early studies in Mexico," Sebelius said.
The US government on Tuesday changed its advisory on school closures related to swine flu, saying that schools were no longer required to close down if students fell ill. Schools were earlier advised to close even if there were suspected flu cases.

"This is not an indication that we know enough about the course that this virus will take," Sebelius cautioned. "We don't know what will happen over the course of the summer and we certainly don't know what will happen when we get into flu season ... later this fall."

Richard Besser, acting director of the CDC, said the "big bump in cases" didn't reflect transmission, but rather that "we're catching up with the testing."

He said the confirmed cases were in patients in the age range of three months to 81 years, with a median age of 16 years. There were 35 hospitalizations in the US, where 62 per cent of those confirmed to have contracted the virus were under 18 years.
In Geneva, the World Health Organization updated the number of global laboratory-confirmed cases of the H1N1 virus to 1,490 human infections in 21 countries.

South Korea on Saturday reported East Asia's second human case of swine flu as other countries in the region sought to minimize the risks of infections by cancelling flights and imposing visa requirements on Mexico, the worst-hit country in the outbreak.

The 51-year-old nun in South Korea confirmed to have the H1N1 strain of the flu has been isolated in hospital since Tuesday after displaying flu symptoms after her return a week ago from Mexico, the Korea Centre for Disease Control and Prevention said.
The announcement came after Hong Kong on Friday night confirmed its first swine-flu case, a 25-year-old Mexican who flew Thursday to the city. He was taken to a hospital isolation ward, and the hotel where he was staying was quarantined.

In response, the governments in mainland China and Taiwan were trying to trace passengers who flew with the Mexican patient as China also suspended direct flights from Mexico, Singapore imposed visa requirements for its citizens and tensions rose among the 240 guests and more than 100 staff quarantined for a week at the Hong Kong hotel.
Indian national Kevin Ireland, 45, said from his room at the Metropark Hotel that the guests began joking Friday night and trying to make light of news that they would be confined to the facility for 24 hours, but he said the atmosphere rapidly deteriorated when they learned from television reports that the quarantine was actually for seven days.

"This morning, there was a Korean gentleman, and he was way off the handle," the father of two from New Delhi said Saturday. "He was screaming and shouting and throwing a tantrum. There is a young couple from the UK. She has been crying incessantly.

"Then there is a South African couple with a 10-month-old baby and their grandmother. The wife was taken away for tests and they are really quite agitated."

He said conditions were exacerbated by "brusque" police, a lack of fresh clothing and linen among the guests, and a dearth of information from authorities.

"Dissemination of any kind of information is lacking," he said. "You ask people questions, and you get no response."

Meanwhile, South-East Asian health ministers prepared for a meeting next week that is to focus on the regional stockpiling of anti-influenza medicines to combat swine flu, officials said Saturday.

Besides including the 10 members of the Association of South-East Asian Nations (Asean), host Thailand has also invited ministers from China, Japan and South Korea to the first such regional meeting on tackling the outbreak of the H1N1 virus.

"We're in discussion with the three countries and still awaiting the response on their level of participation," Thai Foreign Ministry deputy spokesman Thani Thongpaksi said.

Senior-level health officials are to hold a preparatory session Thursday, followed by the ministerial meeting Friday in Bangkok.

The cases reported in South Korea and Hong Kong became Asia's third and fourth after Israel reported it had two cases. All four patients were travellers from Mexico, which late Friday reported it has had 16 deaths from swine flu and 397 infections.

In response, China informed Mexico that it suspended flights by Aeromexico to Shanghai. The government was considering arranging a charter flight to collect Chinese passengers in Mexico who were booked on the first cancelled flight to Shanghai on Sunday, the Foreign Affairs Ministry said in a statement.

The Mexican patient hospitalized in Hong Kong had flown from Mexico to Shanghai before taking the flight to Hong Kong.

Health authorities in the southern Chinese province of Guangdong, which borders Hong Kong, were trying to trace 11 of the 41 local people who arrived in Shanghai on the same flight from Mexico as the infected man, state media said.

The health officials had contacted 30 of the 41 passengers and placed them under a seven-day quarantine, the official Xinhua news agency said.

Likewise, Hong Kong and Taiwan was tracing people who flew with the Mexican patient. Health Minister Yeh Chin-chuan said at a news conference that 26 people, including eight Chinese mainlanders, had flown on the Shanghai-Hong Kong flight before flying on to Taiwan on the same day.

Health authorities had contacted 19 of them but were searching for the seven others, he said.

Among those who have been tracked down, at least six were sent to a hospital for tests to see whether they were infected with the H1N1 flu strain.

Taiwan also issued a travel alert Saturday for Hong Kong and South Korea as Singapore imposed a visa requirement on Mexicans beginning Saturday.

Previously, citizens of Mexico did not need a visa to enter Singapore, which, like Taiwan, has yet to report an infection with the H1N1 virus.

Singapore had already enhanced its surveillance of people arriving from Mexico. Beginning Monday, those with a travel history to Mexico over the past seven days would be quarantined for seven days upon their return to Singapore, the Ministry of Health said.

Ireland said the guests at the Metropark Hotel in Hong Kong regarded their quarantine as an "overreaction."

"We've all got jobs to do, lives to lead and responsibilities," he said.

Health Minister Yeh said H1N1 is bound to enter Taiwan but the public need not panic because the government is fully prepared to fight the virus.

"We are not worried about the virus entering Taiwan," he said. "We are worried about it causing community infection and spreading from one person to another."

South Korea's confirmed infection caused fear to grow that the N1N1 virus could quickly be spread from human to human in South Korea. A 44-year-old woman who lives with the sick nun is being examined on suspicions that she too could have been infected.

The nun, however, has recovered from her fever and other symptoms and might be released from hospital this weekend, authorities said.

Fourteen countries have reported human infections with the new flu strain that has genetic elements from three species - pigs, birds and humans. The only country besides Mexico to report deaths is the United States with one fatality from 141 infections.

Thailand may host an urgent Asean public-health ministers' conference to formulate strategy and action plans for the 10 member countries in fighting the outbreak of swine flu.

The disease is suspected of having already killed as many as 159 people and caused illness among 2,498 others in Mexico.

Public Health Minister Witthaya Kaewparadai yesterday said Asean Secretary-General Surin Pitsuwan had told Prime Minister Abhisit Vejjajiva there should be an Asean conference to discuss the issue as soon as possible.

Surin had asked Cambodia to host the event, but Cambodian Prime Minister Hun Sen suggested Thailand would be a better place to stage such a conference.

Thai health and other authorities played a leading role in mitigating cross-border epidemics like Sars and bird flu. However, the abrupt cancellation of the Asean Summit in Pattaya due to political rioting has damaged the country's reputation.

"We're now discussing where the Asean meeting on swine flu should be held," Witthaya said.

Speaking at the first meeting of a national emergency committee to oversee prevention operations against the swine flu - also known as "Mexican flu" - he added that the conference should take place as soon as next week.

Meanwhile, the committee endorsed a number of actions to prevent the spread of the disease. These are in the fields of public health, livestock management, mitigation and multi-agency collaboration. It also sought a Bt120-million budget to fund health promotion and flu prevention, as well as other public-relation plans.

Chompoonute Cherie Nakornthap, an adviser to the foreign minister, said the Foreign Ministry had asked for a supply of protective face masks for Thais currently in Mexico.

There is also a worst-case plan to evacuate all 105 Thais in Mexico and fly them back to Thailand. Hotel rooms around Suvarnab-humi Airport have been reserved for quarantine purposes should this be necessary.

The government also ordered all hospitals in Bangkok to put on standby 50 airborne-infection isolation rooms to provide emergency medical service for anyone who might be infected with the Mexican flu, even though no cases of infection have yet been reported in Thailand.

The Bangkok Metropolitan Administration announced it would hand out 100,000 face masks to taxi drivers and foreign tourists tomorrow.

Chulalongkorn Hospital vice director Dr Theerapong Charoenvit reported that test results confirmed that a 42-year-old woman who had travelled from Mexico and was admitted to the hospital on Tuesday had contracted seasonal flu - and not swine flu, as was feared earlier.

My Reaction

As swine flu spreads, but not rages, therefore seems to be no lack of reaction by the authorities of both affected and non-affected countries. In the case of mainland china with its 1,200,000,000 people, it is essential that the virus is kept out of china that explains why the Hong Kong authorities were extremely to quarantine the virus that came in from Mexico. The Mexican traveler was immediately taken away, the whole hotel including all its guests was cordoned off and no contact with the outside world was allowed. Even the 50 hotel guests who were staying there, were tracked down by the police and put into quarantine.
While this may seem like an overreaction, on second thought it is not. The risk of the swine flu spreading like wildfire allover china is simply too realistic to ignore. Hong Kong authorities made the right decision to act cautiously and make sure that there was no chance for the virus to spread. It needs to be mentioned that the infected Mexican traveler did not die. There is a feeling of sympathy for the traveler who are still quarantined in the Metropark hotel (where the Mexican stayed); as the number of traveler is as high as 240. However, considering the millions of people who could be affected in case of carelessness, then the number of 240 hotel guests, who are inconvenienced, is not too much of a sacrifice to make. There have been reports of hotel guests becoming enraged about their forced stay at the hotel; this quite understandable but the well being of millions of Chinese citizens is more important than a temporary convenience caused to 240 guests.
Even though china has suspended its flight from Shanghai to Mexico, there is concern that the infected Mexican held in Hong Kong, has spread the virus to people in Shanghai, where he stayed before flying to Hong Kong. Health officials in the Chinese province of Guang Dong, which is right near Hong Kong, are working around the clock to prevent the spreading of the virus, though no infected persons have been found three yet. The cases in South Korea (2), have further alarmed the Chinese authorities. The Korean case concerns a nun who has visited Mexico, and was diagnosed with the virus, as well as the person who went to pick her up at the airport. This has sent shockwaves all over South Korea. Though no infections have been confirmed in Taiwan and Singapore, both countries have been on a high level of alert to stop infected traveler from entering their nations. Strict measures are typical for Singapore, so it is no surprise that the Singapore authorities will quarantine any incoming travelers from Mexico for seven days. This is definitely the right approach as the alternative (meaning: millions of worldwide infections) is simply too horrible to think about.
The interesting point of the swine flu virus is that the infection seems to have a different effect on those victims in Mexico, and those in the USA. The number of fatalities in Mexico is considerably higher, whereas in the USA the infected people only have mild symptoms and the number of fatalities is only 2 deaths. In Mexico, so far 177 people have died. The percentage of deaths compared to the number of infected people is much higher in Mexico than in the USA. It can be concluded from this information that the virus type in Mexico is much more powerful than that in the USA. What should worry most readers is that medical experts have no explanation for this phenomenon. In spite of all the medical advances, this difference in percentage of deaths between Mexico and the USA is still unexplained. The preventing measures to stop the spreading of the virus are in place, though the medical world must be surprised about the nature of this virus, which is an animal-human hybrid virus. Hopes for a quick production of a vaccine are small, because it well take another 3 months for a vaccine to be produced, and another two years to manufacture enough stocks for the whole world to be vaccinated.
A local conference will be held here in Thailand next week, and all ASEAN countries will send representatives to this conference. Thai authorities’ efforts to prevent the disease from entering and spreading in Thailand have so far been effective one woman entering Thailand was first thought to have the virus, but she was misdiagnosed.

Conclusion
The focus on preventive measures is effectively stopping the swine flu disease from spreading too fast. Efforts by authorities worldwide have limited the damage done by the swine flu virus, with only Mexico suffering a high casualty toll. There is no place for despair because it now seems that the disease is not as bad as it was first predicted. It can be concluded that being extremely cautions has significant benefits when fighting a possible pandemic.
